Macro with empty fields

I need to make multiple templates for radiology reports with empty fields that can be choosen by using the tab button. For example,
Examination: CT of the Head
Date: [ ]
History: [ ]
Technique: [ ]
Comparison: [ ]
[Body]
Impression: [ ]
Powerscibe has this function built in but I don't know how to do this with DNS 9.5. I am using a configurable Phillips SpeechMike so I want to map out one of the buttons to the 'TAB' key. Any help is appreciated. Thanks.
