Need advice on replacing Andrea ANC-750 microphone jack - having trouble wokring out the wiring

Hi,
I have an Andrea ANC-750 that I'm needing to repair. I do not have any experience with electronics so took the headset to an electronics store and agreed to let them chop of the original mike jack. They suggest I solder the wires to a new 3.5mm jack or take it to an electronics repair person to have them do it. Only problem is there are five wires in the lead and only 3 connection points on the 3.5mm stereo jack.
I'd really appreciate help regarding the correct connection - i.e. which coloured wires go where on the new 3.5mm jack?
I've taken the headset to an electronics repair person and he is also not sure how to connect it back up.
Thanks in advance for help on this. As the headset is out of warranty I felt I had nothing to lose by trying a relatively inexpensive repair. At the very least I can still use the "headphone" function for listening.

I neglected to mention I no longer have the original microphone 3.5mm jack, the electronic store threw it away, thinking it was no longer needed.
